Surgical Sperm Retrieval is a technique for collecting sperm directly from a man’s testicles or epididymis. It is a procedure carried out under local anaesthetic and/or with sedation. The fertility team may use different methods of SSR depending on your hormone levels and testicular size.
There are a number of reasons why a man may have little or no sperm in his sample. Some have to do with genetic factors whilst others are related to previous illnesses.
For men where no sperm is found in a sample the first step is usually testing, most commonly with blood tests and an ultrasound to try and determine the cause. Following testing your doctor will discuss with you the pros and cons of surgical sperm retrieval and whether it is suitable for you. It can also be indicated for those with a high degree of fragmentation in their sperm DNA.

Azoospermia is a condition in which there’s no measurable sperm in a man’s semen. Sometimes the cause of azoospermia is down to genetics. However it can also occur as a result of sexually transmitted infections, blockages along the reproductive tract, hormonal problems, ejaculation problems or issues with testicular structure or function. Sometimes, the causes are treatable and fertility can be restored.
There is no evidence to show that collecting sperm surgically poses a risk to the health and wellbeing of any children conceived with it. Research has shown that sperm extracted surgically may not fertilise as well, although this could be due to the fact that some sperm is of lower quality.
Wow where do I even begin!? The care, empathy, skill and joy this clinic bring to their patients is really something other clinics should aspire to. We had our IVF round in October 2024 and then a successful first frozen embryo transfer in November 2024 and now have our amazing 9 month only baby boy. From start to finish the levels of care, kindness and compassion from all staff were top notch, everyone we encountered had the most professional yet approachable way about them which made things so easy and comfortable. Myself and my partner were treated so well, we can understand why everyone says they are so nice and supportive because that was our experience too. They were always there when needed, the portal was great and very handy and swift responses for peace of mind. If you are thinking about it just do it! A free consultation changed our lives and after two rounds we have our little boy and we can’t thank the whole team enough.

Me and my partner went for consultation with Emily back in October as we were between clinics and when we came out after speaking with Emily we knew therapie was the right choice. The process from start to finish has been amazing! We had an amazing round with therapie resulting in a good number of frozen embryos and luckily success with our 1st frozen transfer. Therapie made our dreams of a family a reality. I've spoken so highly of therapie throughout my journey. The care has been amazing and looks forward to continuing building our family over the years hopefully with the clinic. Use are a credit to those who struggle ❤️
We had our beautiful twins with help from Thérapie Fertility. The care and attentiveness they showed us along the way was next to none. They take the approach of evidence based only testing/medications etc which we soon realised was the best approach, for us at least, and involved the least stress, cost, pressure etc. They were the best value for this reason and also the general costs of treatment, which eases the stress of IVF somewhat too. As a queer couple we never felt different, always embraced and we would highly recommend to fellow LGBT couples. We see Thérapie Fertility staff as part of our family, our journey, our past and now, our future.
