What is Reciprocal IVF?

Reciprocal IVF is a treatment pathway where one partner undergoes ovarian stimulation and egg collection, while the other partner prepares to carry the pregnancy. The eggs are fertilised with donor sperm, the embryos are created in the laboratory, and one embryo is later transferred to the other partner through frozen embryo transfer.

This is why reciprocal IVF is often described as shared motherhood. It allows both partners to play a physical part in bringing their child into the world.

In our clinic, reciprocal IVF is always planned using frozen transfer rather than fresh transfer. This gives the process more structure, allows time to carefully prepare the uterus, and helps us create the most suitable conditions for the partner carrying the pregnancy. It also means the journey can be managed in a clear, supportive way for both of you.

The Reciprocal IVF process

    Ovarian stimulation & monitoring

    The first stage of Reciprocal IVF is for the partner providing the eggs to take fertility medication, usually as injections, to encourage the ovaries to produce multiple follicles. Each follicle may contain an egg. During this stage, you will attend monitoring appointments, including ultrasound scans and sometimes blood tests, so we can carefully track response and time treatment.

    When the follicles are ready, a trigger injection is given, and an egg collection is planned for 36.5 hours later at the clinic. If you would like a clearer picture of fertility before treatment begins, our fertility assessments can help guide the next steps for both partners.

    Egg retrieval

    Egg retrieval is a short theatre procedure carried out under light sedation. Using ultrasound guidance, the fluid from each follicle in the ovaries is collected and sent directly to the laboratory for the embryology team to examine for eggs. You will receive pain relief, and you should not experience excessive pain during the procedure. Before you go home, we will let you know how many eggs were collected, explain what happens next in the laboratory, and talk you through recovery and aftercare clearly.

    Fertilisation & embryo culture

    While the egg retrieval is taking place, your chosen donor sperm sample is prepared for use in the laboratory. The eggs are fertilised via IVF, and the embryos are then monitored as they develop. They are usually cultured to day 5 or 6, known as the blastocyst stage, before being frozen for later use. Many couples choose a donor with characteristics similar to the partner who is not genetically connected to the embryos, but that decision is completely personal.

    Uterine preparation

    The partner who will carry the pregnancy then begins preparation for transfer. This usually involves medication to help build the uterine lining so it is ready for implantation. We will monitor the lining with ultrasound scans and carefully plan the timing of the transfer. Because this stage happens separately from egg collection, it gives us time to create the most suitable conditions for the partner carrying the pregnancy.

    Frozen embryo transfer

    In reciprocal IVF, we always use frozen embryo transfer rather than a fresh transfer. The embryo is placed into the uterus using a fine catheter in a short, straightforward procedure that does not require anesthesia. Planning the transfer in this way provides greater clarity to the process and allows us to focus on carefully preparing the body. It also provides a smoother pathway for both partners, with each stage occurring at the right time. After your frozen embryo transfer, you will attend the clinic about 10 days later for a blood test to check if you are pregnant.

How do I know if Reciprocal IVF is right for us?

Reciprocal IVF may be right for you if you are a same-sex female couple, or a couple that includes a trans male or non-binary partner, and you would both like to share the physical side of building your family. It can be a particularly meaningful choice when one partner wants to provide the eggs and the other wants to carry the pregnancy. It also allows the couple more choice for the future, as either partner can then become pregnant with the jointly owned embryos.

  • Yes. Reciprocal IVF has been offered in Irish fertility clinics in recent years and is now an important fertility treatment option for same-sex female couples and couples where there is a trans male partner.

  • Yes. If you create embryos through a reciprocal IVF cycle, those embryos belong jointly to both of you. In future, depending on your circumstances and treatment plan, either partner may potentially use those embryos to try to become pregnant.

  • If your child is conceived in an Irish fertility clinic and born in Ireland, both parents can be legal parents from birth. There are some additional steps when registering the birth, and our clinic will provide the documentation you need. If you plan to give birth outside Ireland, local laws may differ, so legal advice is recommended.

  • In Ireland, donor sperm must come from an identifiable donor. This means you do not know the donor personally, but identifying information is recorded on the national donor register. A donor-conceived child can access identifying information about the donor upon reaching adulthood.

  • Yes. Because reciprocal IVF involves donor sperm, implications counselling is required under current Irish legislation. This is an important and supportive part of the process, helping you think about donor conception, future family conversations, and your child's long-term interests.
