A HyCoSy is a fertility test that examines the uterus and fallopian tubes during a transvaginal ultrasound. Using a contrast fluid, it helps assess the uterine cavity and check whether the fallopian tubes are open.

The scan provides valuable information for fertility planning and can help identify factors that may affect conception. The procedure is minimally invasive, usually takes only a little longer than a standard transvaginal ultrasound, and may cause mild discomfort but is not typically painful.

What is a HyCoSy?

HyCoSy stands for hysterosalpingo-contrast-sonography. It is a type of fertility ultrasound used to assess the uterine cavity and fallopian tubes. The test is similar to a transvaginal ultrasound, but with one additional step. A small amount of contrast fluid is gently introduced so it can be seen on the ultrasound. This helps our team check whether the fallopian tubes appear open and whether the uterine cavity looks suitable for pregnancy or fertility treatment.

A HyCoSy scan can provide information that may not be visible on a routine ultrasound alone. It is often recommended for patients who are trying to understand their fertility more clearly or preparing for treatment. An AMH test might also be recommended as part of a wider fertility picture.

    Consultation and history

    Before you have a HyCoSy, you will have a free consultation with one of our fertility nurses. We will talk through your medical history, cycle pattern, previous results, fertility goals and any questions or concerns you may have. This helps us understand whether a HyCoSy scan is suitable for you and how it fits into your wider fertility testing plan.

    HyCoSy procedure

    A HyCoSy procedure is carried out during a transvaginal ultrasound scan. During the procedure, a contrast fluid is gently passed through the uterus so our doctors can check the uterine cavity and fallopian tubes on the scan. Two nurses will be in the room with you throughout, so you feel supported at every stage. The test is minimally invasive and usually painless, though some people may experience mild discomfort or light cramping.

    Results and next steps

    Once your results are ready, they will be shared with you through your patient portal. One of our fertility specialists will talk you through them in detail at your medical consultation, so you understand what they mean for you. Your results may help guide the next step, whether that is further testing, fertility treatment, or simply taking time to consider your options. Whatever you decide, the information can help you move forward with greater clarity and confidence.

We perform a HyCoSy test because the uterus and fallopian tubes play an important role in conception and pregnancy. If the fallopian tubes are blocked or the uterine cavity is affected, this may influence which fertility options are most suitable for you.

A HyCoSy procedure gives our team useful information before treatment begins. It can help us understand whether treatment such as IUI may be appropriate, or whether another option may give you a better chance of success. If your results suggest that the fallopian tubes may be affecting your fertility, our doctors may discuss treatment options such as IVF with you.

The results are always reviewed alongside your wider fertility picture, including your medical history, symptoms, previous results, blood tests and ultrasound findings. However, it does not diagnose all forms of endometriosis. Your results will be reviewed alongside your symptoms, history and any other relevant tests.

  • Our team will advise you on sex after HyCoSy based on your individual situation and how you feel after the test. Some patients may prefer to wait until any spotting or cramping has settled. If you have been given specific aftercare guidance, follow that advice.

  • The difference between HSG and HyCoSy is mainly how the tubes are assessed. HSG usually uses X-ray imaging and dye, while HyCoSy uses ultrasound and contrast fluid. Our team can explain which test is most appropriate based on your circumstances.
